Manifesto

Precision in every code.

Medical coding is more than assigning codes. It requires understanding documentation, anatomy, clinical terminology, cardiovascular procedures, and the details behind every patient encounter.

About me

Ronaq Muhammad

Medical coder — CPC & CCVTC

I am a dedicated medical coder with expertise in CPC and CCVTC, focused on cardiovascular and cardiology coding, clinical documentation, and healthcare data. I bring accuracy, attention to detail, and a deep understanding of anatomy and medical terminology to every case. I also design and build for the web.

Nine layers, nine codes

Heart anatomy · ICD-10-CM · CPT
Whole heart, front view
Heart anatomy
Heart, coronary view
Coronary view
Coronary arteries labeled RCA, LCA, LAD, LCX, OM1, OM2, OM3 and PDA

Coronary arteries

Name the vessel, pick the modifier

RCAStent modifier RCRight coronary artery
LCAStent modifier LMLeft main coronary artery
LADStent modifier LDLeft anterior descending
LCXStent modifier LCLeft circumflex
OM1–3Stent modifier LCObtuse marginals, branches of the LCX
PDAStent modifier RCPosterior descending, usually from the RCA

CPT 92928 covers the first vessel stented. 92929 is the add-on for each additional branch of a major coronary artery.

Worked example

NSTEMI with a stent in the LAD

Chest pain and a positive troponin. Cath shows a 90% proximal LAD blockage, and a drug-eluting stent is placed. Here is how it is coded.

Codes I work with

CPT · ICD-10-CM

CPT (procedures)

93458Left heart catheterization with coronary angiography; left ventriculography when performed
93454Coronary angiography with catheter placement, injection, imaging supervision and interpretation
93460Right and left heart catheterization with coronary angiography
92928PCI with stent placement, single major coronary artery or branch
92941PCI for acute MI with total or subtotal occlusion, single vessel
93306Transthoracic echocardiogram, complete, with spectral and color Doppler
93000Electrocardiogram, 12-lead, with interpretation and report
33208Insertion of permanent pacemaker, atrial and ventricular leads

ICD-10-CM (diagnoses)

I25.10Coronary artery disease of native vessel without angina pectoris
I25.119Coronary artery disease of native vessel with unspecified angina pectoris
I21.4Non-ST elevation myocardial infarction (NSTEMI)
I48.91Atrial fibrillation, unspecified
I50.22Chronic systolic (congestive) heart failure
I10Essential (primary) hypertension
Z95.5Presence of coronary angioplasty implant and graft (stent)
Z95.0Presence of cardiac pacemaker

A sample of common cardiology codes. The correct code always depends on the provider's documentation, the payer and the current code set.

Cases solved by a precise coder

Illustrative scenarios

Cath lab · NSTEMI

A patient with NSTEMI has a left heart cath with coronary angiography, then a drug-eluting stent in the LAD in the same session.

Codes

ICD-10-CM: I21.4, I25.10 · CPT: 92928-LD, plus 93458 with a distinct-procedure modifier when the decision to intervene came from this study.

Why it is right

Reporting 92941 here would be a mistake unless the note documents a total or subtotal occlusion. The coder checks the cath report before choosing the PCI code, which prevents a denial and a compliance risk.

Documentation · chest pain

A known CAD patient is seen for chest pain. The note says "CAD with angina".

Codes

ICD-10-CM: I25.119. Do not add R07.9 (chest pain).

Why it is right

Angina is built into the I25.11- combination code, so a separate symptom code is redundant. Choosing I25.10 would drop the angina and understate severity. The coder reads the note, not just the headline diagnosis.

Echo · medical necessity

A cardiologist interprets a complete echocardiogram in the hospital for a patient with heart failure.

Codes

CPT: 93306-26 · ICD-10-CM: I50.22 (when the note documents chronic systolic HF).

Why it is right

Modifier 26 bills only the physician reading, since the facility bills the technical part. A specific heart failure code supports medical necessity. I50.9 is accepted by many payers but is weaker.
